(1)Students who were absent from school during the spring semester of 2018
(2)Students who studied abroad in the spring semester of 2018
(3)Due to unavoidable circumstances such as the person's illness, etc.
　Students who were unable to receive a medical examination in the spring semester of 2018

October 1 (Mon)-2 (Tue) 13: 30-16: 30　
　　※We will not accept any requests except during this time.

3.Implementation site

Shimonoseki City University Health Consultation Office
※If you are unable to have a medical examination on this schedule, go to the hospital on your own, have a medical examination, and have a medical examination.
　Please submit a medical certificate to the Student Support Group.
　At that time, you will have to pay for the consultation.

[Consultation items]

　Height and weight: For all members
　Sight and blood pressure: For 4th grade undergraduate students and students taking teaching courses.
　Urinalysis and X-rays for all members
　ECG: Only for first-year undergraduate students

When all measurements are completed, please return the personal card to the University Health Consultation Office.
