Request for Performance Disclosure

We will disclose the results of the entrance examination to applicants for the 2023 Shimonoseki City University entrance examination.
The disclosure request period must arrive from May 15, 2023 (Mon) to May 31, 2023.

Billing method

In the case of a window

Please submit the following 12 at Admission Division (2nd floor of Building I).

※You can also give your grade disclosure request at the Admission Division window.
②: University admission ticket

In the case of mailing

Please mail the following 123 to Admission Division.
Please write "Request for Disclosure of Entrance Examination Performance" in red on the envelope surface.

If you lose your admission ticket, you can use the admission ticket (original) for the University Admission Common Test.
③: Reply envelope (Long form No. 3, postage stamp for 434 yen, address specified)

Notes

Only the applicant can claim it.
If the documents are incomplete, we will not be able to accept them.
If you take multiple selections, please submit a request for disclosure of grades for each selection.
However, only one envelope for submission and reply is acceptable.

Method of receipt

It will be issued after the end of the disclosure request period.

In the case of a window

It will be delivered in Admission Division.
Please bring your ID (student ID, driver's license, etc.) to verify your identity.

In the case of mailing

It will be returned with the admission ticket in a reply envelope.
※In case of insufficient postage, etc., we will return it by Yu Packet cash on delivery.

Ranking, total score
(Those who have used the three-part aspiration system are ranked first, total score)

※We will not disclose the following information.

Ranking of those who have completed admission procedures at another university before the announcement of acceptance
Ranking of the 2nd and 3rd subjects who have used the 3rd aspiration system
Ranking and total score of persons subject to special consideration
