Consideration for examination and examination of students

Applicants who need consideration for examination and study due to illness, injury, disability, etc., should consult with our university in advance. Based on the application, we will review and notify you of your consideration.

Depending on the content of the consultation, it may take some time to respond, and it may not be possible to respond by the day of the test and special consideration may not be taken. Please consult as soon as possible.

Application method

After contacting us in advance by telephone, etc., prepare an “Application for Consideration and Study Examination” and attach the required documents and submit it by the specified deadline.
Please download the "Application for Examination and Study" from the following.

1 Contents to fill out the application form

Applicant's name, date of birth, gender, address, telephone number, school of origin
Faculties, departments, etc.
Type and degree of disability
Items to be considered when taking the exam and the considerations you would like when studying
Special considerations taken at the school where they were born, the situation of daily life, etc.

b.Copy of doctor's medical certificate or disability certificate

The doctor's medical certificate will be used as a material for determining the content of consideration, so please ask them to be filled out specifically.

c.University entrance examination "Notice of Decision on Consideration for Examination" ※Only applicable persons

If you have been issued a “Notice of Decision on Consideration for Examination” from the University Entrance Examination Center, please attach a copy.
